Duodenal switch, otherwise called a biliopancreatic diversion with duodenal switch (BPD-DS), is a combination of two more common procedures: the Roux-en-Y gastric bypass and the biliopancreatic diversion. It exists because some patients needed much more help than a gastric bypass could provide and because patients who got the biliopancreatic diversion wanted a more restrictive solution.
It's essentially two different surgeries in one. The first stage is to staple off part of the stomach and create a small pouch, which will be the part of the stomach that can hold food. However, where the Roux-en-Y gastric bypass will reconnect the stomach to a lower part of the intestine (jejunum), the duodenal switch keeps it connected to the upper part (duodenum).
The second stage is to cut the intestine and reroute it. A further lower part of the intestine is reattached to the upper part of the duodenum, creating an alimentary limb through which food can pass. This causes bile and other digestive juices to start digesting food when it gets lower down the intestine. As a result, much less fat is digested and fewer calories can be absorbed.
The surgery combines a restrictive phase (the stomach pouch) and a more extreme malabsorptive phase (the rerouting of the intestines). The result is that patients not only are unable to eat much food in one sitting but a large amount of calories simply don’t get digested and absorbed. As a result, caloric intake is greatly reduced, allowing for drastic weight loss in a short period of time.
